Your Help is Needed with DNP Project

As part of my DNP project, "Restorative Sleep and Spiritual Distress:  Impact of a Primary Care Screening Intervention on Spiritual Health," I am assessing multiple providers' views and practices related to spiritual health.  

To better inform the project, I am casting a wider net to help ascertain the current climate for addressing spiritual health in a variety of settings.  It would be a great asset to this implementation research project if you would be willing to complete the attached questionnaire and return to me.  Return instructions are listed at the bottom of the questionnaire.
